Ensign Group acquires 13 healthcare facilities across three states

The Ensign Group Inc. (NASDAQ: ENSG) acquired operations of 13 healthcare facilities across California, Wisconsin, and Iowa, effective August 1, 2025, according to a company statement.

The California acquisitions include eight facilities comprising 640 skilled nursing beds and 109 assisted living units. The facilities are located in Davis, Fresno, Modesto, Lodi, Turlock, Oxnard, Santa Barbara, and Huntington Park. These acquisitions operate under triple net lease arrangements with affiliates of CareTrust REIT Inc. (NYSE: CTRE) and International Equity Partners.

Three additional California facilities - Shoreline Care Center in Oxnard, Buena Vista Care Center in Santa Barbara, and Huntington Park Nursing Center in Huntington Park - are operating under management agreements pending state regulatory reviews.

In separate transactions on the same date, Ensign acquired Pine Crest Health and Memory Care in Merrill, Wisconsin, and Crystal Heights Care Center in Oskaloosa, Iowa. Subsidiaries of Standard Bearer Healthcare REIT Inc., Ensign's real estate company, acquired the real estate for both facilities.

The acquisitions add over 1,200 operational beds and units to Ensign's portfolio, bringing the company's total to 361 healthcare operations across 17 states, including 47 senior living operations. Ensign subsidiaries own 148 real estate assets.

"As long-term operators, we've never sold a skilled nursing operation," said Barry Port, Ensign's Chief Executive Officer. "Since the very beginning of our organization's 26-year history, we have been serving patients and families in this great state."

The company provides skilled nursing and senior living services, physical and occupational therapies, and other healthcare services across Alabama, Alaska, Arizona, California, Colorado, Idaho, Iowa, Kansas, Nebraska, Nevada, Oregon, South Carolina, Tennessee, Texas, Utah, Washington, and Wisconsin.
